The set can be obtained in the Deep Prison/Lair of Nar Zhul.
First, you'll need to get the Key to the Lair of Nar Zhul, this is done by killing Infernal Protector.
Then the Key to the Outer Sanctum of Nar Zhul is obtained by killing Sanctum Guardian.
You have now reached the lair but still don't have acces to the prison. Kill the mobs around it until a Dungeon Key drops.
To unlock the set quest part enter the prison and kill the Jailor of Nar Zhul until you get the key for the desired spot:

Eye Shaped Jail Key - shield
Diamond Shaped Jail Key - helm
Sword Shaped Jail Key - boots
Demon Head Jail Key - necklace
Skull Shaped Jail Key - chest
Tree Shaped Jail Key - belt
Infinity Shaped Jail Key - blade
Lizard Shaped Jail Key - ring
Heart Shaped Jail Key - pants

Last but not least, get 1 soulgem from each crew raid. Give all the soulgems for 1 set piece.
